Why are the melting point and boiling point different for the same matter?

When a solid substance is heated, it is converted to liquid, and due to further application of heat, it is converted to the gaseous substance. For example, when ice is heated, 
it melts at 0°C temperature and converts to water. And when the temperature of 0°C water is increased, it boils at 100°C temperature and converts to vapor.
